October Coordination of Care Meeting - Connect2Care and Community Discussion

As part of the meeting, we will continue our theme from last month on patient facing applications with a presentation on the Connect2Careconsumer health portal from Holland Bloorview Kids Rehabilitation Hospital. This portal allows clients and their families online access to their health information and supports engagement in their care. Joanne Maxwell, who has a clinical background in Occupational Therapy and is a Project Management Professional has led the implementation of Connect2Care and will be presenting this work for us.

Also, as part of the meeting, we wanted to discuss with you what areas we should focus on as a community. To date, we have had some great presenters from across the country share information and lessons learned about the important projects being planned and implemented here in Canada. We certainly hope to continue with this knowledge sharing, but also felt it would be worthwhile to identify subjects and potential initiatives that might be worthwhile endeavours for the community. Here are a few topics we thought we would discuss:

  • Project List: As an intro, Peter will present a review of some of the Infoway sponsored projects that touch on the Coordination of Care space to provide a backdrop as to the current activity in the area. Participants can further fill us in on other projects that are going on in their respective regions.
  • Discussion of Priorities: What do you see as the priority areas of the Coordination of Care space? Coordination of Care encompasses a broad range of programs including eReferral, discharge summaries, eConsult, EHRs and patient portals; what are seen as the priority programs in your jurisdiction?
  • Potential Collaboration: Given the priority areas, what do you see as potential initiatives for collaboration? This may include the development of pan-Canadian standards or terminology value sets, but other topics such as work on common architectures and related technologies might be worthwhile as well.
  • Meeting content: Clearly, discussion on the previous points may inform the content of future meetings, but we are happy to discuss other ideas as well (e.g. guest speakers, vendor presentations, international projects that might inform work here in Canada, etc..).
