Hello all,

CIHI is interested in knowing what the jurisdictions are using as concepts/value sets in EHR/EMR implementations for the following:

• Care settings or institution types that patients are coming from and going to
• Services patient are referred from/to; and
• Discharge dispositions

Additionally, we’re interested in knowing which jurisdictions have discharge summary, e-referral, or other specifications that may incorporate these concepts.

If you are using pan-Canadian EHR standards for any of the above, we would be interested to know which one.

We’ll appreciate receiving documentations that show any of the above.
