A team in the Netherlands is gathering information on Personal Health Records and asked for the Canadian perspective. They define a Personal Health Record as "a tool to collect, manage and share personal health information, controlled by a patient (or a healthy person), which can contain information about all diseases and is therefore not limited to one health care organization or one disease. It should not be confused with patient portals (which are usually limited to 1 organization) or self management tools (which are usually limited to 1 disease)."
