If it's a jurisdictional patient identifier, the 'type' would be "JHN" (for Jurisdictional Health Number). The same type would be used for Alberta ULI, Ontario PHN, etc. You would also have a URL for the Identifier.system that would be specific to the RAMQ number.

Hi Loyd,

I'm old to HL7 but new to FHIR so I apologize if my comments are too basic. We were just looking at the Patient Resource and trying to understand where we'd put, for instance, the RAMQ Number for here in Quebec. The RAMQ is the unique provincial identifier for the ministry of health.

When I saw the identifiers available in the PS-CA IG for the Patient Resource I was just wondering where we need to put these identifiers of ours since they're not explicitly called out like that.

Based on this response from you, is a 'class' of identifier supposed to then map to a format of that number? For instance, the RAMQ identifier is made up of numbers and letters. Will the class of identifier be a special class for RAMQ, or would we use another field and then specify RAMQ in the identifier.system?

Again, please sorry for my ignorance on this. Just trying to understand all of this better.

Actually, FHIR is pushing back against the creation of this new type because it overlaps with other existing types. Type should identify a 'class' of identifiers that is distinct from other classes. It shouldn't identify a specific number sequence - that's what the Identifier.system does.

I'm not an implementer, but this identifier type does seem too broad/ambiguous. WHICC long ago defined identifier types such as JHNMB, JHNBC, etc. for the health card numbers issued by the various provinces and territories. Also, Terminology Gateway, under the IdentifierType subset, shows an identifier type of JHN | Jurisdictional Health Number (Canada).

Sometimes I worry that this FHIR stuff is unnecessarily "re-inventing the wheel"...

The HL7 Unified Governance Terminology group is currently evaluating the request to add the identifier type of JPID (Jurisdictional Person Identification) to the HL7 v2-0203 Code System . This new type was originally put forward a number of years ago to be used by NamingSystems in the Canadian URI Registry and has since been socialized by the CA Baseline.

However, some of the identifiers previously categorized under this type could map to other values in the table (e.g., Social Beneficiary Number, Permanent Resident Card Number, Treaty Number, etc): Canada Social Insurance Number , I nterim Federal Health Program Identifier , Indigenous and Northern Affairs Canada number , Canada Permanent Resident Card Number

We are seeking input from any implementers of the JPID identifier type to bolster the rationale for its inclusion as a standalone type/assess impact of withdrawing the request.
