New URI Request

It's MOH assigned long term facility identifiers, and diffrent from MOH master numbers.

This looks good to me. I'm just wondering if these identifiers are assigned by Ontario Health and are different from the Facility Master Numbers assigned by Ontario Ministry of Health.
