Be aware that you're not allowed to use the HL7 International logo on any IG that isn't published by HL7 International. (And you can't use the FHIR logo in an IG unless you request specific permission.)
The IG should include a clear explanation of the relationship between this IG and the HL7 SDC IG which supports the same use-cases (plus some others), but uses the FHIR Questionnaire and QuestionnaireResponse resources rather than the IHE custom SDC XML schema.
