High alert medications are often implicated in harmful medication incidents.

How does your organization handle the prescribing, transcribing, dispensing, administration and monitoring of high alert medications to prevent harm?

ISMP in the U.S. has just released a tool to help you assess your medication system.
The Medication Safety Self Assessment for High Alert Medications is now available. Click on the title to go to the web page.

They have also had a webinar, which is recorded to highlight the key features.


The workbook can be download for a manual assessment, they have an excel template to track your results or you can login to their online self-assessment form (and it looks like it is not limited to US only).

Nonetheless, check out the workbook link and review it. I'm sure you will discover some gaps in your organization processes that can be implemented to improve medication safety.
