Hi Everyone,
I came across this interesting program, and wanted to share this information for those managing highly complex patients (including chronic pain patients). Each participant has an opportunity to present a case to a specialist team - what a great way to collaborate on your patient's care!
Would be interested to hear if there are similar programs in other provinces/territories. Please share any details you may have!
Kind regards,
Seema
ECHO links primary care providers from across the province with each other and with a specialist team during weekly videoconference sessions. Participants gain knowledge, skills and support for management of complex, chronic conditions. The 2 hour sessions are CME accredited and include a short didactic followed by de-identified case presentations by ECHO participants. Case presenters are provided with a list of recommendations and all participants gain from case discussions and shared knowledge.
ECHO sessions can be easily accessed using a web based link. Sessions may be attended by any member of the inter-professional team including Physicians, Nurse Practitioners, Physician Assistants, and Pharmacists. ECHO Ontario is funded by the MOHLTC. There is no fee to register.
