Let's increase the momentum for medication safety awareness during CPSI's Patient Safety Week, starting October 30th.

This year's theme is on medications: "Take with Questions". It is linking with the 5 Questions to Ask About your Medications.

Organizations across Canada are being encouraged to endorse the 5 Questions to Ask , by adding their logo to the document and incorporating the engagement of patients in medication discussions.

Let's get the conversations started about medications! Improved safety will be the outcome.

From my clinical work as a home visiting pharmacist - open dialogue with patients and caregivers reveals medication related problems 95% of the time. It's a very important conversation to initiate.
