Here is a success story where a hospital listened to feedback from patient and their families.
“As we started looking at that planning for these two projects, we’d gotten a lot of feedback from our patients and families that the documents they received upon discharge were a little bit lacking,”
They worked with their Patient and Family Advisory council - and actually listened when they said that the layout and format of the medication list was not patient and family friendly!
Kudos to this group.

Does anyone have a story to share about how your organization worked with patients and caregivers to design medication lists that were useful? Please share!
