This alert describes medication errors that are attributed to the use of IT solutions for medication management in hospitals.

Errors include: over-riding alerts (or disable them!), workarounds due to workflow problems, free texting, incorrect weights entered, downtime procedures

They offer a multitude of strategies to improve medication safety with HIT.

CPOE errors were over 50%.

To help identify HIT based errors, they added a reporting prompt "Did Health IT cause or contribute to this event?"
