It really helps us to realize that successful medication management is over and above medication reconciliation. But all steps must be complete in order to truly meet the needs of the patient.

Selkirk Mental Health Centre is currently working with a vendor called BSharp Technologies to develop an electronic MedRec Module which interfaces directly with the Order Entry, eMAR and pharmacy module. Within the eMedRec module, the MedRec on admission, MedRec - Return from Pass and MedRec- Discharge are separate processes which result in a viewable/printable record of the completed Reconciliation by the physician. This module was designed by pharmacists, nurses and physicians based on our standardized paper process and communicated to the vendor in order to develop a true electronic MedRec process. To date, the eMedRec on admission is complete and is being trialed on an Area which has converted to the electronic Order Entry and eMAR. The eMedRec - Return from Pass, eMedRec- Discharge as well as an electronic Patient Medication Review are currently being developed within the module. I will keep the group posted on when this work will be complete.

This is a document from Australia.
They have broken down the steps in their overall medication management and identified how electronic systems are used and impact each stage.

Has anyone in Canada done this for our medication management system? If so, please share.
If not - there is an opportunity here.
