The Patient Summary document is a “health record extract comprising a standardized collection of clinical and contextual information (retrospective, concurrent, prospective) that provides a snapshot in time of a subject of care’s health information and healthcare.” (ISO/TR 12773-1:2009).
It contains essential clinical information that enables a common view and understanding of the patient within the circle of care and may be used by health care providers to support:
- Transition and transfer of patients across the continuum of care.
- Coordination and collaboration around a patient’s care in the context of Ontario Health Teams.
- Safe provision of care in unexpected or unscheduled medical situations (e.g., emergency or urgent care).
