Welcome to the Canadian Hospital at Home Community!

This is a collaborative space dedicated to the advancement and implementation of hospital at home models in Canada. Hospital at Home is an acute clinical service that brings hospital care to selected individuals in their homes or nursing homes[1]. It substitutes for acute inpatient hospital care by providing staff, equipment, technologies, medication, and skills typically found in hospitals directly to patients in their home environment[2].

Our community is a hub for healthcare providers, leaders, patients, caregivers, policy-makers enthusiastic about this cutting edge model of acute home-based care. As we build this community, we aim to to connect, share insights, and advocate for the growth of hospital at home services across the nation.

Our Objectives

  • To connect individuals and organizations passionate about hospital at home.
  • To share learnings and best practices for in-home acute care delivery.
  • To advocate for policies and systems that support the hospital at home model.
  • To disseminate knowledge and drive innovation in the field of home healthcare.
