This post includes a proposed baseline for the pan-Canadian Patient Summary (PS) use cases. The baseline was developed on our interpretation of the material provided by each jurisdiction, which has also been incorporated into the Knowledge Base.

• Does this list of use cases meet the minimum baseline requirements within your jurisdiction?
• Are there any use cases missing that you think should be included in the baseline?

For each use case, we have tagged the jurisdictions that have identified the use case as being a priority or an item on their roadmap.

UC-01: HCP Sends PS to EHR

A Health Care Provider (e.g., Primary Care Provider, HIS Physician) creates or updates a Patient Summary (PS), using their clinical system (e.g., EMR, Hospital Information system) and shares it with the EHR Repository so that other authorized Health Care Providers will have access to the Patient Summary.

UC-02: HCP Receives Notification of New PS in EHR

Health Care Provider receives a notification of Patient Summary updates available in the EHR Repository, with option to view and consume the Patient Summary.

UC-03: Patient Views/Obtains Personal PS

Patient accesses and views their own Patient Summary in a clinical system such as a Patient Portal (e.g., Alberta's MyHealth Records).

UC-04 - HCP Sends PS to Another HCP as part of a Clinical Workflow (e.g., eReferral)

Health Care Provider creates a Patient Summary, using their clinical system, and shares with another Health Care Provider via a clinical workflow (e.g., eReferral, eConsult, secure message).

UC-05: Patient Presents PS to HCP in Another Jurisdiction
Patient presents a copy of their Patient Summary to a Health Care Provider in another jurisdiction, or any Health Care Provider treating the patient who does not have access to the Patient Summary, who consumes the Patient Summary.

UC-06 - HCP requests PS from EHR on Demand

Health Care Provider requests a Patient Summary from the EHR, which is created on-demand at the time of request from one or more source systems for viewing and consumption.
