Hello all,

A friendly reminder about the eReferral working group meeting today. This will probably be a quick and low key meeting as I am stuff catching up post FHIR North and post-vacation, but would like to touch base to work together to plan out the next few months of focus for FHIR eReferrals.

Also, we've updated the FHIR server funcitonaity post-FHIR-north to be able to handle more data fields, so would like to connect with those who would like to try out the integration again.

Also, will be discussing an novel alternate method to implementing SMART on FHIR that allows the sender to POST data to the app (instead of the app GETting from the launching system).

As mentioned today on the eReferral workstream call, Ontario is hosting a webinar and full day workshop to collect input on the proposed future state architecture for the Ontario eReferral ecosystem. The details and how to register are provided below. Registration closes April 19.
Unspoken in the invite is that we intend to leverage the set of FHIR eReferral APIs that this workstream is developing to enable the flow of referrals across systems throughout the lifecycle of a referral.

As part of our engagement plan for electronic referrals (eReferrals), we are hosting a 1.5 hour webinar to provide an overview of the proposed Provincial eReferral Conceptual Architecture developed in support of the Provincial eReferral Strategy.
The participants will be presented with a high-level integration design of how various systems in the provincial eReferral ecosystem will work together to enable healthcare providers to participate in the full eReferral lifecycle. Presentation materials will be shared in advance of the meeting.

Target audience:
Public and private solution delivery providers that offer eReferral capabilities (sending or receiving), vendors, innovators, IT planners, and regional delivery partners/LHINs with an expressed interest in cross-system eReferral interoperability.

Referrals in Ontario:
eReferral systems enable health care providers to digitally initiate, receive, manage and track referrals. eReferrals benefit multiple stakeholders across Ontario’s healthcare system: patients benefit from more efficient access to specialized care and support services as well as enhanced safety and quality of care as they transition through the care continuum; health service providers benefit from improved communication and a more streamlined referral process; and health system planners benefit by having access to consistent and accurate referral data to inform their planning. The value that an eReferral solution brings to the end-to-end referral process increases when it can interoperate with many other systems to expand the number and type of service providers that can be referred to, leverage existing clinical data, reduce duplication and streamline workflow.

Provincial eReferral Strategy:
In 2015, the PanLHIN Referral Management Working Group was created as a forum for knowledge sharing and collaboration to enable LHINs to increase efficiencies and advance provincial standardization of referrals.
In August 2017, the PanLHIN Referral Management Working Group submitted a Statement of Work to the Ministry of Health and Long-Term Care to develop a Provincial eReferral Strategy. This strategy is to provide the foundation to guide digital health partners to advance the implementation of eReferrals, in a coordinated manner, in order to achieve improved access and better patient outcomes across the province.

The strategy includes completion of the following supporting parallel work:
• Design of Architecture and Interoperability Guidelines
• EMR eReferral Integration/Interoperability Requirements
 Current State Inventory
In March 2018, funding was received from the ministry to commence the strategy.

Purpose: Provide an overview of the proposed Provincial eReferral Conceptual Architecture in advance of the May 2, 2018 in-person workshop

Following the April 23rd webinar, we will invite all stakeholders to a 1 day in-person workshop (May 2, 2018) held in Toronto, to walkthrough the requirements in detail and solicit your input.
The following topics will be discussed:
• Current state business and clinical workflows (e.g. referral communications, provider searches in service catalogue, patient public portals) for various referral pathways including Primary Care, Specialists, Acute, Community, etc.;
• Proposed authentication and authorization models for users and systems;
• Proposed referral data flows between point of service systems (i.e., EMRs, HIS) and referral systems;
• Centralized service catalogue/directory of providers and their services;
• Central referral repository to provide end-to-end referral visibility and reporting; and
• Architecture alignment with the provincial eReferral guiding principles.

This week we'll be going over how to get started with a basic integration using the FHIR specification, in preparation for the FHIR North Code camp event happening on April 26th. We will show an example of submitting an eReferral, relaying an eReferral, and launching an eReferral via SMART on FHIR.

Backgrounder for FHIR North Code Camp eReferral track:

Tim walked through the end-to-end interactions between the actors. One system can support multiple actors

2 ways to connect with an eReferral FHIR server:
· Either ahead of time and then demo it at FHIR North… and connect with others
· Build at FHIR North itself

Focus is on either traditional POST or SMART on FHIR
The 3 flows include send/post/relay and the acknowledgement/response from the receiver;
Out of scope is any more involved interactions e.g. request for (additional) information

Flow 2.3 - the Caredove server will be the 'sender' of the relay referral and those that have a 'check' in the Flow 2.3 column will be the receivers. The payload for Flow 2.1 is the same as for Flow 2.2, with the addition of a Task resource

It is completely acceptable to simply take the 'human narrative' payload and copy it into a 'Notes' section of the receiving system

Flow 2.4 - SMART on FHIR
The sender will share a Patient and Practitioner resource along with OAuth2 authorization and then the receiver will present a UI. The sender does not need to have a UI running; a browser is sufficient.

Completion % values at the top of the various pages of the eReferral spec indicates the level of confidence of the content. If it is above 50%, there is strong confidence but requires tweaking. This is one of the goals of the FHIR North Code Camp eReferral exercise. Those with less than 50% indicates that the technical considerations have a lot of confidence but the content has not be published. Overall, the approach has been 'through the ringer' with the international FHIR community so there is less likelihood of wholesale changes but potential 'gotchas' that need to be worked out.
