There are also a number of tools available for patients that you may be interested in including a free "My MedRec" smartphone app to help patients maintain a list of their medications and immunizations. They are also able to record when they take their medication (good for monitoring compliance) and other parameters such as blood pressure and allergies. Patients are able to email their profile to their healthcare provider from within the app.
We have recently developed 5 Questions to Ask About Your Medications. It is a tool designed to help patients and their caregivers ask questions about their medications when they are interacting with healthcare providers so that they obtain the information they need to use their medications safety. The tool was designed by patients and healthcare providers and is in use across Canada. It is available in both English and French.
I work in France around patient empowerment tools to decrease quality and safety issues of prescription medication. I have not found anywhere so far any tool to be used by patients themselves, enabling them to capture and maintain information over time about their own treatments, and receive some alerts in case of conciliation issues, or obvious errors, etc.
Has anyone ever seen /heard about tools used by patients themselves (in complement/synergy to the role of the pharmacists and GPs) ?
