The Med Safety Exchange by ISMP Canada and CPSI is part of how Canada is meeting the WHO Medication Safety Challenge, aiming to reduce medication errors by 50% of the next 5 years.

They have completed their first series, delivering 6 monthly webinars, sharing the learnings from actual medication errors, with representatives from the affected organization, hospitals or community practice sites speaking up.

The webinar in February really highlighted the medication risks with pediatric patients and provided recommendations for any professional dealing with this population. There was an example that highlighted our passive dependency of technology to perform calculations - but with a clinician using their rational and cognitive skills, was able to prevent an incident in a child.

This webinar also looked at how performing Multi-incident analysis (MIA) using a collection of incidents with narrative documentation is a tremendous learning tool. Did you know that ISMP Canada offers workshops on how to do an MIA? Check out: www.ismp-canada.org/education/MIA.htm

Two medication incident cases were presented, with the first one illustrating how technology can contribute to medication errors.
The learnings illustrated that establishing policy and communication pathways at the forefront could prevent errors.
