Updated! Medication Reconciliation Getting Started kits

Hello Members,

The Getting Started Kits for MedRec in Acute Care and Long Term Care have been recently updated to incorporate newly published evidence. They can be found on the ISMP Canada website, under Medication Reconciliation. English and French versions are available.
