This is a recent publication that holds a lot of value in helping organizations implement medication reconciliation processes (either paper or electronic). Nurses, pharmacists and physicians were surveyed on the various steps within medication reconciliation to discern each professional's role.
This study concludes: The three health care professions recognize the value of medication reconciliation and want to see it implemented in the hospital, yet there is a lack of agreement with regard to roles and responsibilities of each profession within the process.

The results of Table 3 illustrates the discord between the professions.
