Hello Diagnostic Imaging Community,

If you have a chance, there are a couple of topics you could think about before the DI call this Friday, March 24th, regarding FEM:

1.) Current proposed use cases:

a.) radiology priors (the "most common" FEM use case)
b.) radiology reporting performed separately from the imaging institution

What other use cases, if any, do we want to include in the scope of the proposal? This is a key question....

2.) Focus on installed base systems:

In 2013, one of our primary assumptions was that "we need to make the installed base systems work better without having to change them. Do not force sw dev onto all of the existing systems, just make them work better. Make the FEM actor in the middle the "intelligent" actor."

Is this still an underlying premise in this proposal? I believe so, but needs discussion and consensus.

3.) Transactions/technology:

In the CHI XDS Affinity Domain Implementation Guide published in 2013, we focused on the following transactions:

-DICOM C-Store/C-Move, all objects including SR (Structured reporting as DICOM IOD)
-HL7 v2 ORU
-XDS.b
-XDS.b-I
-IHE PIX

This makes sense given the year 2013.

Today, there are more options available including PIXm, DICOM web services, and FHIR. We need to discuss whether or not to expand the technology choices beyond 2013. I would propose that we do, but this clearly needs discussion and consensus.

4.) Focus on reusing existing IHE transactions.

I would very much like to reuse as many existing IHE transactions as possible. (that is the key points of IHE, actually.) There is a new HL7 v2.5.1 report (ORU) message under development which I would like to discuss because it has different assumptions than we did in the 2013 XDS Implementation Guide. This is critical for many installed base systems.

5.) I would like to re-review the Radiology Priors use case sequence diagram just to get everyone back onto the same page. It is still quite appropriate, actually. :)

6.) Then we also need to look at the doodle poll dates....and decide.
