Call for Interest: Care Plan Template Development

The Coordination of Care Community is soliciting interest from members related to the collaborative development of a Care Plan Template that can be leveraged by jurisdictions. If you are interested in joining a working group focused on Care Plans please respond to this post and watch the Forums for ongoing activity.

We are also interested in gathering sample Care Plans from jurisdictions that could be used as a starting place or potentially contribute to the template sections. If you have a Care Plan example that you could share with the group please let us know!
