Hi Diane and Finnie,

I would love to join this group. Currently at Fraser Health, we are working on a minimum data-set type document for an interprofessional, collaborative care plan. Would love to share the engagement that has happened thus far and our steps to move forward. It would also be great to learn from others and see how we can adjust our course accordingly.
