Help Advance eMedRec Solutions: the Paper to Electronic MedRec Implementation Toolkit
Join Lisa Sever, Consultant Pharmacist with ISMP Canada, as she shares updates on the Paper to Electronic MedRec Implementation Toolkit.
Medication reconciliation (MedRec) is intended to prevent harm arising from ineffective communication of medication information as patients transition across healthcare settings. Although it is a critical component of providing safe care, it is complex and challenging to implement system-wide. Canadian providers have clearly indicated that technology to support the MedRec process is essential for successful implementation across the healthcare system.
In 2014, with support from Canada Health Infoway, a toolkit was developed by researchers at the University of Victoria, in collaboration with AE Informatics Inc., the Institute for Safe Medication Practices Canada, and the Canadian Patient Safety Institute to support organizations in their migration from a paper-based system to an electronic system for MedRec.
As part of the Joint Statement of Action to Address the Opioid Crisis, and a commitment to provide an updated national electronic medication reconciliation toolkit , work is underway to update the Paper to Electronic MedRec Implementation Toolkit.
